These forms are provided as a courtesy to parents who either are in the process of enrolling their child at Christ Lutheran Children's Center or already have children enrolled but wish to update their child's information. These forms are in the Adobe Acrobat format. If you have not installed the Adobe Acrobat reading software on your computer, please visit the Adobe web site for more details on downloading and installing it.

Instructions:
1. To download the file, either click on the file name to open the form directly in your web browser, or right-click on the file name and select "Save As..." from the menu to save a copy of the form directly to a location on your computer.
2. To fill out the form, either print the form and fill out the fields by hand, or you can type directly in to the form fields and print that (this is the preferred method).
